Who Can Use the Patient Referral Network?
Our platform was developed with two key audiences in mind:
General Practitioners
Clinics not equipped to provide long-term care to patients with chronic conditions can use our platform to quickly connect with the right local resource.
Acute Care Centres
Our platform provides a superior method of patient referral by focusing on three key points:
• Simplifying the process of referring patients on discharge with chronic conditions to local specialists
• Improving communication between GIMRAC, GP/IM hospitalists and local specialists at transition points in care
• Lowering wait times in the emergency department and freeing up beds by reducing unnecessary hospital admissions
